Purpose:To evaluate peeling of internal limiting membrane(ILM)as adjunct to vitreous surgery for macular pucker. Cases and Methods:The series comprised 21 eyes of seconary epiretinal membrane,or macular pucker, during a 3-year period. It was secondary to vitrectomy for retinal detachment 6 eyes, scleral buckling 6 eyes, vitrectomy for diabetic retinopathy 2 eyes, ocualar sarcoidosis 2 eyes, intraocular foreign body 2 eyes and others. Ten eyes received intentional ILM peeling. Eleven eyes received peeling of epiretinal membrane only. Cases were followed up for 12 months after surgery. Results:Epiretinal membrane recurred in 1 out of 10 eyes receiving ILM peeling and in 6 out of 11 eyes receiving peeling of epiretinal membrane. Visual acuity improved by 2 lines or more in 7 eyes in the former group and in 4 eyes in the latter. Metamorphopsia improved in 6 eyes in the former group and in 2 eyes in the latter. Conclusion:Intentional ILM peeling results in better outcome during vitreous surgery for secondary epiretinal membrane.
