Abstract. Purpose:To compare the outcome of wavefront-guided LASIK with and without preoperative surgeon's adjustment. Cases and Method:This study involved 86 eyes treated by wavefront-guided LASIK for myopia. The series comprised 36 eyes that received adjustment and 50 eyes that did not. WaveScan(R)(VISX)was used for wavefront analysis. Excimer laser was performed with VISX Star S4(R). Results:The difference between subjective and wavefront-guided spherical power averaged 0.48 diopters in adjustment group. Postoperative uncorrected visual acuity averaged 1.49 in adjustment and 1.38 in non-adjustment group. The difference was not significant. Subjective spherical power and spherical equivalent were -0.01 and -0.13 diopters in adjustment group and -0.13 and -0.22 diopters in non-adjustment group. Refraction in adjustment group was significantly closer to emmetropia. Conclusion:Wavefront-guided LASIK with preoperative surgeon's adjustment resulted in better outcome in eyes with differences between subjective and wavefront-guided spherical power.
