遠視LASIK後のIOL度数予測を行った1例

Prediction of intraocular lens power for an eye after LASIK for hyperopia

　67歳女性の左眼白内障に対して手術を計画した。幼児期から約10.5Dの遠視があり，21か月前にLASIKが行われ，約5Dの矯正効果が得られていた。手術直前には＋9Dの遠視があった。眼内レンズの度数決定に3つの方法を検討した。Clinical history法を利用して求めた角膜屈折値を使ってSRK/T式で計算する方法，LASIKが行われている角膜をOrbscan^(R)で測定して得られる角膜前後面像を利用し光線追跡法で求める方法，同じく角膜前後面像を利用しSRK/T式で求める方法である。以上のうち，clinical history法が適当であると判断し手術を行った。術後6か月の時点で等価球面度数-1.25Dに相当する矯正で0.8の視力が得られた。

　Cataract surgery was planned for the left eye of a 67-year-old woman. She had had hyperopia of about＋10.5 diopters（D）. She had received laser in situ keratomileusis（LASIK）21months before. After temporary improvement of hyperopia by about 5D,her left eye had hyperopia of＋9D before cataract surgery. We compared the following 3methods in determining the power of intraocular lens（IOL）. Use of SRK/T formula using keratometric data obtained by clinical history method. Use of ray-tracing analysis of anterior and posterior corneal power as measured by Orbscan^(R). Use of same data processed by SRK/T formula. We performed cataract surgery using the first method. She obtained visual acuity of 0.8 by an equivalent spherical correction of-1.25D.
