鉄欠乏性貧血の若年女性に発症した網膜中心静脈閉塞と網膜中心動脈分枝閉塞の合併症例

Simultaneous occlusion of central retinal vein and branch retinal artery in a young female with iron-deficiency anemia

要約　目的：片眼に網膜動静脈閉塞症が発症し，並存する鉄欠乏性貧血に対する治療が奏効した若年女性の報告。症例：24歳女性が右眼の視力低下で受診した。1か月前から一過性の右眼視野欠損が頻発していた。初診時の矯正視力は右0.02，左1.2であり，右眼眼底に網膜静脈の拡張と蛇行，Roth斑様の出血，乳頭の下鼻側に浮腫があった。蛍光眼底造影所見と合わせ，網膜中心静脈閉塞症と網膜動脈分枝閉塞症の併発と診断した。内科的には治療が必要な全身疾患はなかったが，食思不振があり，身長163cm，体重35kgと痩せていた。血液検査で血清鉄が46μg/dlと低値であった。結果：血栓溶解療法でいったんは改善したが再び増悪した。第10病日から鉄剤を経口投与し，その12日後に眼底所見が改善し，視力が1.0になった。結論：鉄欠乏性貧血が基礎にある網膜循環障害には，貧血の治療が奏効することがある。

Abstract.　Purpose：To report a young woman with simultaneous occlusion of retinal vein and artery that responded favorably to treatment for iron-deficiency anemia. Case：A 24-year-old female presented with visual loss in her right eye. She had had repeated episodes of amaurosis since one month before. Corrected visual acuity was 0.02 right and 1.2 left. Funduscopic and fluorescein angiographic findings were compatible with simultaneous occlusion of central retinal vein and branch retinal artery. She had anorexia,was 163cm tall and weighed 35kg. She had low level of serum iron. Findings：Systemic thrombolytic therapy was futile. Fundus findings and visual acuity rapidly improved after she was given peroral iron preparations. Conclusion：Treatment for anemia may be effective for retinal circulatory disorder associated with iron-deficiency anemia.
