網膜中心静脈閉塞症から網膜動脈分枝閉塞症をきたした鉄欠乏性貧血の1例

Branch retinal artery occlusion in a case of central vein occlusion and iron-deficiency anemia

目的：経過中に網膜動脈分枝閉塞症が発症した網膜中心静脈閉塞症と鉄欠乏性貧血の症例の報告。症例と経過：47歳女性の右眼に一過性の視朦が生じた。視力は1.2で，切迫型網膜中心静脈閉塞症と診断した。その5週後に再度の発作があり，右眼視力が0.04に低下した。眼底に網膜動脈分枝閉塞症の典型的な所見があった。内科で鉄欠乏性貧血が発見され鉄剤を経口投与した。4か月後に眼底所見は正常化し視力は1.0に回復した。結論：本症例では，鉄欠乏性貧血により切迫型網膜中心静脈閉塞症が生じ，乳頭の浮腫などにより網膜動脈分枝閉塞症が発症した可能性がある。

Purpose：To report a case of central retinal vein occlusion and iron-deficiency anemia followed by branch retinal artery occlusion. Case and Findings：A 47-year-old female noted blurring of vision in her right eye. Her visual acuity was 1.2 in either eye. Funduscopy showed findings compatible with impending central vein occlusion. She had another episode of blurring in the same eye with visual acuity of 0.04. Her right eye showed typical findings of branch retinal artery occlusion. Systemic studies led to detection of iron-deficiency anemia. After 4months of treatment with peroral iron,the fundus findings became normalized with the visual acuity of 1.0. Conclusion：There is a possibility that iron-deficiency anemia induced central retinal vein occlusion. Optic disc edema would have resulted in branch retinal artery in this patient.
