網膜中心静脈閉塞に毛様網膜動脈閉塞が併発した鉄欠乏性貧血症の1例

Simulataneous occlusions of cilioretinal artery and central retinal vein in a case of iron-deficiency anemia

　網膜中心静脈閉塞に毛様網膜動脈閉塞を併発した44歳女性を報告した。内科的精査の結果，鉄欠乏性貧血を認めたため，血管強化剤，循環改善剤および血栓溶解剤（アボキナーゼ®）の点滴に加え鉄剤の投与を行った。2か月半後，貧血の改善とともに検眼鏡的眼底所見は正常化した。

　網膜中心静脈閉塞の発症には，鉄欠乏性貧血による低酸素状態が関与し，解剖学的に脆弱な毛様網膜動脈はその網膜中心静脈閉塞による二次的な影響を受けやすいと思われる。

A 44-year-old female noted blurred vision in her left eye since 3 months before. Sudden visual field defect developed 3 days before visiting us. She had undergone subtotal gastrectomy for peptic ulcer 14 years before. We diagnosed her left eye as central retinal vein occlusion with cilioretinal artery occlu-sion. We detected iron-deficiency anemia, appar-ently secondary to gastrectomy. Systemic treat-ments with iron preparations and thrombolytic agents resulted in improvements in fundus findings 10 weeks later.
