 It is easy to end to solution of only medical problems(M),in patient records that we have done since before and enter main appeal and symptom,views,explanation,plan etc. Even if we do the most effective treatment to patients that patients requesting it,it is not necessarily able to become the medical treatment that obtained understanding and agreement. Making an effort as I understand regarding the background of the problem where the patient has,by recording mental(P:physiological problem)and also social(S:social problem)problems in addition to medical problems in Kato Eye Clinic,I am making use of it for the care of patients. We are thinking that we can practice(POMR:problem oriented medical record)such problem inclination style clinical record entry law,by using the electric patient records even in busy outside origin medical treatment. Being able to record it with the letter that is easy to read many information,because we are recording medical treatments by using the personal computer I am easy to explain even the medical treatment contents in the past. We thought that we can understand patient oneself more deeply not only diseases. However,the training is necessary to practice POMR. By using the electric patient record even in outside origin medical treatment,long-term POMR becomes possible and the mutual understanding with patients deepened. The consciousness reform to the medical treatment that gives treatment to the part of the care of patients,from the medical treatment that attaches diagnosis to the symptom as a result,and do treatment was done.
