シクロホスファミド内服を試みた壊死性強膜炎2例

Two cases of necrotizing scleritis treated by systemic cyclophosphamide

副腎皮質ステロイド薬が無効な壊死性強膜炎の2症例にシクロホスファミド内服が奏効した。1例は54歳女性で，左眼に壊死性強膜炎が7か月前からあり，ステロイド薬の点眼と内服は無効であった。1日量100mgのシクロホスファミド内服で2年5か月後に治癒した。他の1例は68歳女性で，20年前からの関節リウマチに対しプレドニゾロン内服を行っていた。両眼に壊死性強膜炎，右眼に角膜穿孔があった。シクロホスファミド内服と羊膜移植で4か月後に継続的な治癒が得られた。

Systemic cyclophosphamide was effective in two cases of necrotizing scleritis after corticosteroid proved futile. One case was a 54-year-old female with necrotizing scleritis in her left eye since 7 months before. She had been treated by systemic and topical corticosteroid with no sign of improvement. Peroral cyclophosphamide at the daily dosis of 100 mg induced lasting cure after 2 years 5 months. The other case,a 68-year-old female,had been receiving prednisolone for rheumatoid arthritis for 20 years. Both eyes had necrotizing slceritis. The right eye showed threatening corneal perforation. Cure was obtained by systemic cyclophosphamide and transplantation of amniotic membrane.
