翼状片と結膜弛緩症同時手術例

Simultaneous surgery for pterygium and conjunctivochalasis

目的：結膜弛緩症を伴う翼状片に対し同時手術を行った症例の報告。症例と方法：結膜弛緩症を伴う翼状片7例7眼を対象とした。年齢は平均72歳であり，術後2年間の経過観察を行った。翼状片単独手術では遊離結膜弁を12時方向の結膜に作製しているが，結膜弛緩症で切除した球結膜を翼状片切除後の弁移植に使用した。結果：7例中6例で良好な術後結果を得た。1例では結膜移植弁の縫合糸が術後に切れ，12時方向の球結膜を切除して追加移植を行い，以後は経過良好である。結論：今回の術式は結膜弛緩症を伴う翼状片に有効である。

Purpose：To report the method and outcome of simultaneous surgery for pterygium and conjunctivochalasis. Cases and Method：This retrospective study involved 7 eyes of 7 cases with pterygium and conjunctivochalasis. Their age averaged 72 years. All the cases were followed up for 2 years or longer. We used free conjunctival flap obtained during surgery for conjunctivochalasis to cover the defect after excision of pterygium. Results：Cure was obtained in 6 out of 7 cases. One case needed repeat transplantation of bulbar conjunctiva after postoperative loosening of suture thread fixating the flap. The case is doing fine thereafter. Conclusion：The present surgical method is useful for pterygium with conjunctivochalasis.
