We evaluated the role of flomoxef sodium (FMOX) in the prophylaxis of infection after cataract surgery. A group of 33 cataract patients received 1.6g of FMOX intravenously 30 to 300 minutes before surgery. Its aqueous concentra-tion was measured during surgery. Maximum level of 2.1±1.7 μg/ml was obtained 150 minutes after infusion. Another group of 146 patients received 10 to 20 mg of FMOX subconjunctivally 30 to 300 minutes before surgery. Maximum aqueous concentration ranged from 53.4 ± 22.6 μg/ml to 110.4±67.9 μg/ml dependimg on the dosage. Side effects were absent throughout the 179 cases. In another group of 100 cataract eyes, 9 species of bacteria were isolated in the conjunctival sac in 21 eyes. The majority of these bacteria had minimum inhibitory concentration (MIC) of 0.5 μg/mlor less. Staphylococcus epidermidis isolated in the surgical room had MIC of 4.0 μg/ml or less. These findings show that subconjunctival FMOX is more effective than intravenous one for the prophylaxis of postoperative endophthalmitis.
