急速に進行した類上皮型の脈絡膜悪性黒色腫の1例

A case of epithelioid type choroidal malignant melanoma with rapid progression

　急速に進行した悪性黒色腫の1例を報告する。68歳女性が右眼視力低下で受診した。矯正視力は右0.5，左1.2で，右眼の視神経乳頭近くに色素沈着がある3乳頭径大の隆起性病変があった。蛍光造影の初期には低蛍光，後期には過蛍光と低蛍光が混在した。6か月間に急速に大きくなり，硝子体出血が併発した。超音波検査で茸状の隆起性病変であり，悪性黒色腫が疑われた。術中の生検で悪性黒色腫の診断が確定し眼球を摘出した。腫瘍はブルッフ膜経由で網膜に浸潤するメラニン色素が豊富な類上皮型悪性黒色腫であった。視神経への浸潤はなく，以後1年間の経過は順調である。類上皮型脈絡膜悪性黒色腫は頻度は低いが，急速に進行することがある。

　Purpose：To describe a case of rapidly progressive malignant melanoma. Case and Findings：A 68-year-old woman presented with corrected visual acuity of 0.5 right and 1.2 left. Her right eye showed an elevated and pigmented lesion of 3 disc diameters near the optic disc. The tumor rapidly enlarged during the next 6months and developed vitreous hemorrhage. Ultrasonography showed the tumor to be mushroom-shaped. Biopsy during surgery showed malignant melanoma and led to enucleation. The tumor invaded the retina through the Bruch's membrane. It was identified as epithelioid type malignant melanoma. Conclusion：Epithelioid type choroidal malignant melanoma is infrequent. This case illustrates that it may undergo rapid progression.
