We performed cataract surgery on the right eye of a 82-year-old female. She had been receiving medication for diabetes mellitus since 13 years,injection of insulin since 10 years,and treatment for systemic hypertension since 2months. She had received uneventful cataract surgery in her left eye 10 years before. Surgery for her right eye was performed under tetracaine instillation. After phacoemulsification-aspiration,IOL was fixated in the ciliary sulcus. While the corneoscleral wound was being sutured about 45minutes after start of surgery,the iris and the vitreous started to prolapse through the corneoscleral wound,resulting in expulsive hemorrhage. The systemic blood pressure was elevated to 250/110mmHg. The eyeball had lost vision and had to be enucleated because of intractable pain 5 weeks after surgery. The choroid showed extensive hemorrhagic detachment up to the site of vortex veins. Hemorrhagic detachment was more pronounced in the superior quadrant,suggesting that the expulsive hemorrhage started from the superior choroidal segment.
