多剤併用時におけるブナゾシンのラタノプロストへの併用効果

Addition of topical bunazosin to latanoprostin multiple medical treatment for glaucoma

　目的：ラタノプロストを含む多剤治療眼でのブナゾシン追加による眼圧下降効果の検討。対象と方法：チモロール0.5％，ラタノプロスト0.005％，ドルゾラミド1％を3か月以上点眼している原発開放隅角緑内障50名50眼が対象。25眼にブナゾシン0.01％を追加し，他の25眼には追加せず対照群とした。眼圧，自覚症状，他覚所見を3か月間観察した。結果：実験開始前の眼圧は，ブナゾシン投与群が22.4±1.8mmHg，対象群が22.2±2.0mmHgで有意差はなかった。ブナゾシン投与群の眼圧は，4週後で-1.4±2.1mmHg（p＝0.0046），12週後で-2.0±1.7mmHg（p＝0.0008）と有意に下降した。2週後に比較し，12週後の眼圧下降はさらに有意であった（p＝0.0344）。対照群の眼圧は，実験開始後のいずれの時点でも，開始前と比較して有意な変化はなかった。結論：多剤使用眼では，ブナゾシン追加によって有意に眼圧が下降した。2週後の眼圧下降は比較的小さく，時間の経過とともに眼圧がさらに有意に下降したことから，ブナゾシン併用の効果判定には4週間以上の投与が必要である。

　Purpose：To evaluate the ocular hypotensive effect of bunazosin hydrochloride 0.01％added to multiple drug therapy including latanoprost 0.005％. Cases and Method：A total of 50 eyes of 50 patients with primary open-angle glaucoma were enrolled in the prospective study. They had been receiving timolol 0.5％,latanoprost 0.005％and dorzolamide 1％for at least 3months. Randomly selected 25 eyes were additionally given bunazosin 0.01％,and the other 25 eyes served as control. They were followed up for 3months. Findings：Baseline intraocular pressure（IOP）averaged 22.4±1.8mmHg in bunazosin group and 22.2±2.0mmHg in control group. IOP in bunazosin group decreased significantly by 1.4±2.1mmHg（p＝0.0046）after 4 weeks and by 2.0±1.7mmHg（p＝0.0008）after 12 weeks. IOP after 12 weeks also decreased significantly from IOP after 2 weeks（p＝0.0344）. IOP in the control group showed no significant differences from the baseline throughout. Conclusion：Additional bunazosin induced significant IOP reductions after 4 to 12 weeks. Since the hypotensive effect was small after 2 weeks,more than 4 weeks are necessary to evaluate the full effect of additional bunazosin.
