ラタノプロストの併用薬剤としてのチモロールXE，ブリンゾラミドおよびブナゾシンの降圧効果の比較

要約　目的：ラタノプロストに併用するチモロールXE，ブリンゾラミド，ブナゾシンの眼圧下降効果の比較。対象と方法：眼圧が21mmHg以上の開放隅角緑内障で，6か月以上のラタノプロスト点眼でさらに眼圧下降が必要と判断された45例45眼を対象とした。年齢は42～81歳（平均70歳）である。ラタノプロスト点眼開始前の平均眼圧は22.9mmHg，点眼後の平均眼圧は18.0mmHgであった。各15眼にチモロールXE，ブリンゾラミド，ブナゾシンの追加点眼を行い，眼圧の変化を12週間観察した。点眼開始前は3群間に眼圧の有意差はなかった。結果：12週間後の眼圧下降は，チモロールXE群2.0±0.8mmHg，ブリンゾラミド群2.2±0.7mmHg，ブナゾシン群1.4±0.8mmHgで，前2群での眼圧下降はブナゾシン群よりも有意に大きかった（チモロールXE：p＝0.0494，ブリンゾラミド：p＝0.00693）。結論：ラタノプロストで加療中の開放隅角緑内障にチモロールXEまたはブリンゾラミドを12週間追加点眼すると，ブナゾシン点眼よりも有意に大きく眼圧が下降する。

Abstract.　Purpose：To compare the hypotensive effect of timolol XE,brinzolamide and bunazosin as adjunct to latanoprost. Cases and Method：This prospective study was made on 45 eyes of 45 patients of open-angle glaucoma that had intraocular pressure（IOP）of 21 mmHg before treatment and that needed further hypotensive effect after topical latanoprost for 6 months or over. Patients were aged 42 to 81 years,average 70 years. IOP before and after latanoprost averaged 22.9 mmHg and 18.0 mmHg respectively. Cases were randomly divided into 3 groups of 15 eyes each. Each group received timolol XE,brinzolamaide or bunazosin for 12 weeks as adjunct to latanoprost. Results：IOP decreased in each group after 12 weeks. The decrease averaged 2.0±0.8 mmHg after timolol XE,2.2±0.7 mmHg after brinzolamide,and 1.4±0.8 mmHg after bunazosin. Hypotensive effect was greater after timolol XE and brinzolamide than bunazosin（p＝0.0494 for timolol XE,p＝0.00693 for brinzolamide）. Conclusion：Instillation of timolol XE,brinzolamide or bunazosin for 12 weeks induces decreased IOP as adjunct to latanoprost. Hypotensive effect is greater after timolol XE or brinzolamide than bunazosin.
