Purpose:To report a case of systemic hypertension and repeated ischemic optic neuropathy. Case and Findings:A 61-year-old female had been receiving hemodialysis for chronic nephritis. Her corrected visual acuity was 1.0 right and 0.9 left when first seen by us. Optic atrophy was present in the left fundus. Four years later,her right visual acuity decreased to 0.6. The optic disc in the right eye was pale and edematous. Fluorescein angiography showed dye leakage. Another 7 years later,the visual acuity decreased to 0.1 right and 0.9 left. Both eyes showed horizontal hemianopia. We diagnosed her with hypertensive choroidopathy and acute ischemic optic neuropathy. Spontaneous recovery set in,resulting in visual acuity of 0.7 right and 1.0 left 6months later. Fundus findings improved but visual field defect persisted. Conclusion:This case shows that acute ischemic optic neuropathy may show various clinical manifestations,particularly when associated with hemodialysis or systemic hypertension.
