Abstract. Purpose:To report long-term course of anterior ischemic optic neuropathy(AION)in 5 patients with good visual acuity. Cases:This retrospective study was made on 5 eyes of 5 cases of AION who could be followed up for 3 years or longer and who maintained visual acuity of 1.0 throughout. The series comprised 3 males and 2 females. Their age ranged from 52 to 79 years, average 64 years. Findings and Clinical Course:The optic disc was segmentally swollen in 2 eyes and showed segmental pallor or hypofluorescence in 3 eyes. Perimetry showed segmental decrease of sensitivity in all the eyes. Mean and pattern standard deviations by Humphrey perimeter and frequency doubling technology showed deviations by 2dB or less throughout the course of observation. The disc cupping remained unchanged in all the eyes. Conclusion:There is a benign type of AION characterized by good visual acuity and stable clinical findings for 3 years or longer.
