Purpose:To report the effect of early vitrectomy in a case of endogenous endophthalmitis. Case:A 42-year-old male noted sudden blurring in his left eye. He had had fever after receiving drainage of pus from gingival abscess 10 days before. He was diagnosed with uveitis elsewhere. Liver abscess was detected by computed tomography(CT). When referred to us the following day,his corrected visual acuity of 1.5 right and 0.2 left. The left eye showed hypopyon and dense vitreous opacity. We performed vitrectomy the same day. Extensive retinal necrosis with subretinal abscess was detected intraoperatively. Results:Necrotic retinal area started to turn into scar tissue 2 weeks after surgery. Left visual acuity improved to 1.2 5 weeks after surgery. Conclusion:This case illustrates that early vitrectomy may be effective for endogenous endophthalmitis.
