網膜静脈分枝閉塞症と裂孔原性網膜剝離を伴った眼トキソカラ症の1例

Branch retinal vein occlusion and rhegmatogenous retinal detachment in a case of ocular toxocariasis

　59歳男性の右眼に視力低下を伴う飛蚊症が突発し，その5日後に受診した。左右眼とも視力は1.0であった。右眼に硝子体出血があり，上方の網膜に滲出斑と網膜から前方に連なる出血があった。1か月後に硝子体出血は消退していたが，右眼上方に馬蹄形裂孔と，その周囲に網膜剝離があった。蛍光眼底造影で，上鼻側静脈が乳頭のすぐ上方で閉塞し，それよりも周辺眼底に無灌流領域があった。硝子体手術，眼内光凝固と，SF₆ガス注入で網膜剝離は治癒した。患者は飼い犬と寝食をともにしていた。トキソプラズマの血清抗体値は陰性であったが，犬回虫の抗体が陽性であった。手術の際に，滲出斑に囲まれた囊胞様所見が2か所あった。滲出のある網膜部位を切除し，犬回虫（toxocara canis）と思われる虫体が検出された。眼内に犬回虫の幼虫が移行し，これが網膜静脈分枝閉塞症と網膜硝子体癒着による網膜裂孔になり，網膜剝離に至ったと解釈した。背景因子がない症例の眼内に滲出性病変をみたときには，動物との接触を疑う必要がある。

　A 59-year-old male developed seeing flies and blurring in his right eye 5 days before. His visual acuity was 20/20 in either eye. The right eye showed vitreous hemorrhage,retinal exudates,and a strand of hemorrhage adherent to the retina. When seen one month later,the vitreous hemorrhage had disappeared. A horseshoe-tear was present in the superior periphery surrounded by retinal detachment. Fluorescein angiography showed occlusion of superior temoral vein without hemorrhage. The retinal detachment healed after vitrectomy,photocoagulation,an tamponade with SF₆. The patient kept a dog as pet in his house. While toxoplasma antibody was negative,a piece of toxocara canis was identified in the specimen form the retina. Intraocular migration of toxocara larva might have induced branch retinal vein occlusion and vitreoretinal adhesion leading to retinal tear and detachment. This case illustrates that intimate contact with pet animal may result in toxocara intection and ocular complications.
