We reviewed 179 eyes of 167 patients who received penetrating keratoplasty at an institution during a 4-year period. The series comprised 68 eyes of 65males and 111 eyes of 102 females. The ages averaged 69 years. Keratoplasty was performed for corneal leukoma 92 eyes(51%),bullous keratopathy 55 eyes(31%),keratoconus 12 eyes(7%),as second keratoplasty 14 eyes(8%),and others. After follow-up for 2 years or over,the graft remained transparent in 149 eyes(83.2%)in the whole series,in 73% of eyes with bullous keratopathy,and in 43% of eyes after second keratoplasty. Graft failure resulted in 30 eyes,including 10 eyes due to rejection. The corneal endothelial cell density decreased by 24.1% after one year and by 41.9% after two years. The rate of decrease was significantly greater in eyes receiving second keratoplasty after one year and in eyes with bullous keratopathy after 2 years.
