Abstract. Purpose:To report marked hyperopia after cataract surgery in an eye with prior free-flap refractive keratectomy. Case:A 56-year-old woman presented with decreased visual acuity since 2 years before. She had received free-flap refractive surgery 8 years before. Her best-corrected visual acuity was 0.3 in either eye. Keratometry showed a flat area lower-temporal to the corneal apex. Cataract surgery was performed on the left eye. As keratometer was useless, K value was set at 36.09 diopter by calculation from curvature in the central cornea. The axial length was 32.12mm. Prospective power of intraocular lens(IOL)was -0.59 diopter after modified SRK formula. The left eye showed hyperopia by +10.0 diopter after surgery. Replacement of IOL with target value of -9.94D resulted in visual acuity of 0.8 when corrected by +1.0D. Conclusion:Accurate calculation of K value may be difficult in an eye with prior refractive surgery.
