網膜中心動静脈閉塞症を呈した抗リン脂質抗体症候群の1例

Occlusion of central retinal artery and vein in a case of antiphospholipid syndrome

　網膜中心動脈閉塞症と網膜中心静脈閉塞症の合併という，きわめて稀な病態を呈した抗リン脂質抗体症候群の1症例を報告する。症例は58歳女性で，左眼の急激な視力低下を主訴に近医より紹介され，即座に線溶療法開始するも改善せず，光覚弁となった。抗核抗体，抗DNA抗体陽性等よりSLEが疑われたが診断基準を満たさず，ループスアンチコアグラント，抗力ルジオリピン抗体陽性より抗リン脂質抗体症候群と診断された。これまでSLEや分類不能膠原病に網膜血管閉塞症を合併した症例は数多く報告されているが，今後このような症例には本症候群を念頭に入れて検査，加療する必要があると思われる。

A 58-year-old female presented with simultaneous occlusion of central retinal artery and vein in her left eye. Laboratory studies showed elevated titers for lupus anticoagulant and anticardiolipin antibodies in the serum, leading to the diagnosis of primary antiphospholipid syndrome. This case illustrates that this syndrome may underlie occlusive retinal angiopathy.
