開放隅角緑内障112例112眼を対象に,ハンフリー視野計で測定した視野障害と,神経線維アナライザー(GDxVCC)で測定した神経線維層厚との関係を検索した。年齢は20~80歳(平均55歳)であり,矯正視力は0.8以上であった。健常者21眼を対照とした。緑内障の93眼(83%)で両検査法による進行象限が一致した。一致しなかった19眼中13眼(68%)では,ハンフリー視野計での非進行象限に対応するGDxVCC象限に強い鼻側変化があった。残りの6眼(32%)では,GDxVCCで測定した上方2象限と下方2象限での神経線維層厚に差がなかった。GDxVCCによる緑内障眼の検索で,視野に変化がない眼底部位での神経線維層厚の異常を検出できる可能性がある。
We evaluated 112 eyes of 112 patients of open-angle glaucoma regarding visual field and retinal nerve fiber layer(RNFL)thickness. Their ages ranged from 20 to 80 years,average 55 years. All the eyes had visual acuity of 0.8 or better. Visual field was measured by Humphery Field Analyzer. RNFL thickness was measured by GDxVCC,a commercially available scanning laser polarimeter. Another series of 21 eyes of 21 healthy persons served as control. Findings by the two methods were congruous in 93 glaucoma eyes(83%). In 13 out of the remaining 19 eyes(68%),GDxVCC showed advanced RNFL thinning in the nasal sector of perimetrically unaffected quadrants. There was no difference in RNFL thickness between the superior and inferior hemisphere in the other 6 eyes(32%). The findings show that RNFL thinning may be detected by GDxVCC in fundus areas with apparently normal perimetric findings.
