網膜色素上皮下出血から生じた網膜色素上皮裂孔の2例

Two cases of retinal pigment epithelial tear secondary to subretinal pigment epithelial hemorrhage

出血性網膜色素上皮剝離（出血性RPE剝離）に網膜色素上皮裂孔（RPE裂孔）が続発した2症例を経験した。59歳女性と77歳男性で，いずれも片眼に発症した。初診時の矯正視力はそれぞれ0.8と0.15であった。1例は-4.0D，他の1例は-2.75Dの近視であった。2症例とも，当初は少量の出血性RPE剝離と網膜下出血があり，短期間で出血性RPE剝離が拡大しRPE裂孔になった。裂孔の色素上皮弁は2例とも中心窩側にあった。最終的に黄斑下の線維性瘢痕が形成され，視力はそれぞれ0.07と0.1になった。RPE裂孔形成の急性期の診断では蛍光眼底造影の価値は低く，検眼鏡による観察が重要であった。

Two cases presented with subretinal pigment epithelial hemorrhage. Both cases,a female aged 59 years and a male aged 77 years,were unilaterally involved. Visual acuity in the affected eye was 0.8 and 0.15 when corrected by －4.0D and －2.75D respectively. The initially small-sized hemorrhage rapidly enlarged and induced formation of retinal pigment epithelial tear. The flap of torn retinal pigment epithelium was located towards the fovea in both cases. These lesions turned into fibrous scar resulting in visual acuity of 0.07 and 0.1 respectively. Ophthalmoscopy was more useful in the diagnosis of retinal pigment epithelial tear than angiographic procedures.
