In order to understand the pathogenesis of in-creased intracranial pressure, the concept of thedriving pressure (DP) from the intracranial vascularsystem was proposed. The DP consists of the trans-mission of atrial pressure (ADP) and venous pres-sure (VP). It is well known that when the animalsdied, the intracranial pressure decreased to theatmospheric pressure. This means that the DP isimportant in maintaining the static pressure of thecerebrospinal fluid. The theoretical equation of theintracranial pressure was expressed as follows.

ICP =ADP+VP (1)

ADP is expressed as (2).

ADP= η(BP-VP) (2)

The pressure transmission rate (η) was calculatedas the ratio of the CSF pressure to the systemicarterial blood pressure in the experimental andclinical cases. In the normal CSF pressure, 72 wasvery small (2.0×10-2), while in the severe intra-cranial hypertension, η approached to 1.

The concept of the driving pressure was usefulto analyze the static intracranial pressure and thepulse pressure.
