巨大脳結核腫の1治験例

SUCCESSFUL TREATMENT OF A CASE WITH LARGE BRAIN TUBERCULOMA

I．緒言

　近年，抗結核治療剤の進歩により，頭蓋内結核感染症は，きわめて少なくなつたが，特に脳結核腫はほとんどみられなくなり，かつその診断も脳腫瘍と誤診される場合が多い。最近，われわれは7年前肺結核雑結核性髄膜炎の既往歴をもつ16才男子の左側頭葉，後頭葉さらに，頭蓋底にまで拡がつている巨大な脳結核腫を，低体温麻酔下，Valladares氏開頭法によつて全摘したので，若干の文献的考察を加え報告する。

On August 1963, a patient, 16 years old male was admitted to our clinic with a chief complaint of epileptic status. Seven years ago he had pulmonary tuberculosis and meningitis tuberculosa.

His X-ray film of the skull showed a diffuse cal-cified shadow expanding to left temporal, occipital and skull base. Operation was performed by Val-ladares' approach and a large tuberculoma (160 gram weight) was removed.

The post operative course was uneventful and the patient discharged 90 days after surgical operation.
