Recent advances in the understanding of the mechanism of headache have improved the treatment of chronic headache. The great auricular neurotomy was reported by Mason, W. R. (1953) as a new procedure of the sur-gical treatment for tic douleureux. In our clinic this operation has been carried out for various type of chronic headache.

The mechanism of the action of the great auricular neurotomy may be thought as foll-ows: The posterior and anterior branches of the great auricular nerve join the facial nerve, and the great auricular nerve is purly sensory, then some of its fibtes join the facial nerve and carried into the all parts of the face with it. The afferent pathways of pain from the face, head and neck, must reach consciousness via somatic sensory nerves, which supply these regions, therefore, the section of the great auricular nerve is the interruption of one of the afferent pathways.

This procedure was carried out on 26 pati-ents suffering from the posttraumatic head-ache, atypical facial pain, migraine, tension headache, hypertension etc. In about 81%, 21 cases, we were successful in relieving the pain, and this result was much better than expected.
