I．はじめに

　耳下腺良性腫瘍に対する手術として腫瘍の核出術は禁忌とされ，通常，浅葉切除術，深葉切除術，葉部分切除術が行われている^1,2）。これらの術式では顔面神経の温存がポイントとされているが，大耳介神経，下顎後静脈の温存についてはあまり取り上げられていない。

　14例の耳下腺良性腫瘍に対して，minimally invasive surgeryの観点からこれらの温存を試みた切除術を施行したので，その術式を中心に述べる。

　We tried to resect benign parotid tumors in 14 cases preserving the posterior branch of the great auricular nerve and the retromandibular vein. There were 3 cases of deep lobe tumor,and 11 cases of superficial lobe tumor.

　In 13 of 14 cases the posterior branch was preserved.

　The retromandibular vein was preserved as a funiculus when dissecting the lower portion of the parotid gland. In 12 of 14 cases the retromandibular vein was preserved. The procedure was not so difficult and not time comsuming,and the preservation of these nerve and vein should be attempted for all cases of benign parotid gland tumors.
