Lithium and Extrapyramidal Side Effects

I．はじめに

　近年，躁うつ病の長期lithium carbonate（以下，lithiumと略す）療法中に錐体外路症状を呈した症例が報告されるようになった^{1〜3,6〜11,13,14）}。しかし管見の範囲内では本邦での報告例はみあたらない。

　我々は，lithium治療中の入院・外来患者の錐体外路症状の発現頻度を調査し，21例中1例にこれを認めた。この1例について抗パーキンソン病薬投与，およびlithium減量による治療を試み，錐体外路症状に対する効果の観察を行ったので報告する。

　Extrapyramidal side effects (EPS) without elevated serum lithium levels have been observed recently in patients with affective disorders under long-term lithium therapy. The authors examined, from a neurological point of view, 21 patients who had received lithium therapy for at least 2 months without concomitant neuroleptics. Of the 21 patients, EPS was eliminated in only one case (4.8%). The case: A female inpatient of 71 suffering from bipolar disorder had received neuroleptics and tricyclics successively for three years. Both drugs were discontinued on admission and lithium treatment started with 600mg/day.
