てんかん非治療下での強制正常化現象を呈した1例

A Case of Epilepsy with “Forced Normalization” without Antiepileptic Treatment

I．はじめに

　てんかん患者において幻覚，妄想，強迫行為など多彩な精神症状が出現し，この時脳波上のてんかん性律動が消失し，背景脳波も律動異常が消失してα律動となる現象を1953年Landoltは強制正常化現象（forced normalization）と名付けた^1）。彼はこの現象が抗てんかん剤非投与下でもみられることを指摘しているが，最近の報告例をみると抗てんかん剤投与下の場合が圧倒的に多く，非投与下の例は稀である^5）。非治療下での本現象の検討はてんかんと精神症状のantagonism^7）を追究する上でも重要と考えられる。

　We reported a case of an epileptic who showed “forced normalization” on EEG without receiving antiepileptic treatment. The patient was a female, and was 11 years old at the onset of the disease (1974). Antiepileptic drugs gradually decreased the epileptic attacks, which were suppressed completely from 1979 to 1982. On 10-11-1985 she presented restlessness, aggression, auditory hallucinations, delusions without regularly receiving antiepileptics, and consulted a psychiatric hospital. EEG showed 5~6HZ activities under this psychotic state.
