心筋障害を合併したSchönlein-Henoch紫斑病の1例

A Case of Schönlein-Henoch Purpura with Myocardial Complications

　心筋障害を合併したSchönlein-Henoch紫斑病の1例を経験した．患者は64歳，男性．約10kgの体重増加を伴う全身浮腫および起座呼吸とともに，心拡大ならびに肺うっ血が認められ，同時に関節痛，血尿および蛋白尿が出現した．精査の結果，中等度の腎機能障害が明らかとなり，Schönlein-Henoch紫斑病によるmesangial proliferative glomerulonephritisによるものと判明した．しかし，本症例ではさらに左室腔拡大ならびに左心機能低下があり，右心室の心内膜下心筋生検の結果，心筋障害が確認された．Schönlein-Henoch紫斑病が心筋障害を合併することは極めてまれであるが，抗原抗体反応による免疫機転が冠動脈末梢枝にも波及し，微小血管炎の結果，心筋障害が発生したものと考えられた．

A rare case of Schonlein-Henoch pupura with myocardial complications was reported. A 64 year-old man was admitted to our hospital for further examina-tion of hematuria, associated with arthralgia, proteinur-ia, and previous episodes of anasarca and orthopnea. The examinations revealed moderate renal dysfunction due to mesangial proliferative glomerulonephritis sec-ondary to Schonlein-Henoch purpura. In addition, the patient proved to have cardiac dilatation and dysfunc-tion of the left ventricle.
