ドプラーカテーテルによる大伏在静脈グラフト血流量の測定

Measurement of blood flow of saphenous vein graft with Doppler catheter

　ドプラーカテーテルを冠動脈内に直接挿入し，その冠動脈内の血流速を測定することが可能となった^1,2）。しかし，血流量の測定はなおもその精度に疑いがもたれており，有効な手段とは考えられていない。しかし大動脈—冠動脈バイパス術に使用する大伏在静脈グラフトにおいては，側枝を持たずかなりの大きさの内径を有しているため，冠動脈とは異なりグラフト内の血流量の測定が可能であると考えられる。そこでわれわれは大伏在静脈グラフトを用いて施行した大腿動脈—膝窩動脈バイパス術中に，ドプラーカテーテルおよび電磁流量計にて同時にグラフト流量を測定し，ドプラーカテーテルにて測定した血流量の精度の確認を試みた。その上で実際に大動脈—冠動脈バイパス術後の症例においてその大伏在静脈グラフトの血流量の測定を試みた。

We investigated the measurement of blood flow of saphenous vien graft by Doppler Catheter. Under operation of femoro-popliteal bypass graftings (3 cases with 5 grafts of arteriosclerosis obliterans) with reversed saphenous vein grafts, blood flow of saphenous vein graft was measured. At the same time, blood flow of saphenous vein graft was measur-ed by electromagnetic flowmeter. And the values obtained by Doppler Catheter (y) and those obtained by electromagnetic flowmeter (x) were compared. As a result, a good corelationship (y=0. 752×-14.9, r=0.836, p=0, n=20) was obtained. In conclusion, we suggest that the values of saphenous vein graft can be measured by Doppler Catheter. We actually measured the blood flow of saphenous vein graft with CABG. The measurement was thought to be possible.
