植込み後6年目に発生したrunaway pacemakerの一症例

A case report of runaway pacemaker after six years of implantation

はじめに

　近年，人工ペースメーカーは改良が加えられ性能が良くなってきており，合併症の頻度は減少してきている。しかし新旧の機種が混在している現在を考えると，その合併症を熟知していることが必要である。それらのひとつであるrunaway pacemakerは，頻度は少ないが早急に対処しないと極めて危険な合併症である。最近われわれは，ペースメーカー植込み後6年目にrunaway現象をおこして来院し，直ちにgeneratorの摘出交換を行って救い得た症例を経験した。その症例を呈示し，若干の文献的考察を加えて報告する。

A 61-year-old man was treated by pacemaker implantation due to complete A-V block six years ago. Pacemaker had been working well until he complained tachycardia, headache and palpitation in July 1982. When he visited our hospital ECG record showed the pacing rate of 115/min and "runaway pacemaker" was diagnosed. The cause of runaway pacing seemed to be exhaustion of battery power. The generator was exchanged as soon as he was admitted and the postoperative course is good.

Runaway pacemaker is a rare complication after pacemaker implantation but so critical that the promt treatment is necessary.
