A case of runaway pacemaker

　Runaway pacemakerは直接死に結びつく最も重大なペースメーカーの故障である。最近我々は完全房室ブロックによるAdams-Stokes発作に対してペースメーカーを植え込み，術後4日目にrunaway pacemakerをおこした症例を経験したので，故障したペースメーカー本体の分析検討結果と併せてここに報告する。

Runaway pacemakers are an uncommon com-plication of permanent cardiac pacing. A 70-year-old woman with reccurent syncopal attack due to advanced A-V block has treated with AMTI permanent pacemaker (Model DB-401) implantation. 4 days after implantation, she suddenly collapsed. An electrocardiogram demon-strated the pacemaker spike at a variable rate between 125 to 1000 per minute, and 150 to 160 beats per minutes of ventricular activation. Prompt surgical change of the pulse generator was performed and the patient was recovered. Testing of failed pulse generator by the manu-facture revealed battery failure due to leakage of condenser unit.
