RI angiographyによる虚血性心疾患の拡張期心動態の評価—Fourier解析による検討

Assessment of diastolic properties of left ventricle by Fourier analysis of global left ventricular volume curve obtained from RI angiography

　虚血性心疾患では限局性の左室壁虚血により左室全体の動態特性が変化すると考えられる^1〜3）。特に，虚血による拡張期心動態の変化は収縮期心動態に変化が現れる以前に出現するため^4,5），その検索はきわめて重要である。左室容積曲線（left ventricular volume curve）には左室動態特性を示す情報が含まれているので，近年，心臓核医学的方法により得られる左室容積曲線から種々の指標を導き出し，各種心疾患の拡張期特性が検討されるようになってきた^6〜10）。また，regional left ventricular　volume curveにFourier解析を行い心室のfunctional　imageを作製する研究も盛んになされている^11〜13）。本研究では，RI angiographyより得られるglobal left　ventricular volume curveにFourier解析を行い，何次まで近似すれば一様近似が得られるかを検討し，また，新しい指標により拡張期特性を評価し，虚血性心疾患の拡張期心動態を検討した。

To assess the resting left ventricular diastolic preperties in the patients with ischemic heart disease, we analyzed global left ventricular volume curve obtained by radionuclide angiocardiography with Fourier analysis in 72 patients. Analysis in 22 patients revealed that 4 harmonics approximation in the Fourier series could be within Poisson noise. Correlation coefficient between original volume curve and 2 harmonics approximation was 0.987. We made a resurge curve with the value of first harmonic longitudinally and 2 harmonics appro-ximation horizontally.
