気管支喘息の発作中に急死した4剖検例

Four autopsy cases of sudden death during asthmatic attacks

　従来，気管支喘息発作によって死に至ることは少ないとされていたが，近年，喘息発作による死亡は必ずしも稀でないことが認識されるようになった。特に，我が国における喘息死亡率は人口10万人に対して10人前後であり，米国に比して著しく高いことが報告されている^1）。

　喘息死の原因についてみると，光井らの報告^2）では喘息発作中の死亡例540例のうらちほぼ3分の2に当る350例が窒息死とされている。事実，喘息発作時の窒息による死亡はきわめて重大な問題であるが，この中に死因の詳らかでない症例が少くない。その中にはさまざまな原因が考えられるが，その原因の一つとして何らかの心機能の失調も考慮されるべきであろう。

Four cases of an asthmatic death were report-ed with the autopsy findings. All cases weresuddenly deteriorated and dead during the moderate degree of an asthmatic attack. Clinical courses of the three cases were closely followed by physicians or nurses from the deterioration to the death. In all cases, the first sign of the deterioration was a loss of consciousness and faintness of the peripheral pulse.
