冠動脈および右気管支動脈より肺内への側副血行路を認めた大動脈炎症候群の1例

A case of aortitis syndrome with the collateral circulation from the coronary artery and the right bronchial artery to the lung

　大動脈炎症候群は若年女性に好発する動脈炎で，大動脈とその主分枝動脈，肺動脈などに比較的限局した血管壁肥厚を生じ，内腔の狭窄，閉塞性病変をきたす疾患である。病理組織学的には血管壁に非特異性増殖性炎と内膜の二次性線維化や血栓形成による変化がみられ，病変の拡がりと炎症の消長によって多彩な臨床症状を呈する^1）〜4）。近年，本症候群における冠動脈病変が注目されており^5）〜9），また肺動脈病変に伴う肺内側副血行路として気管支動脈の発達^10），冠動脈よりの側副血行が報告^5）されている。われわれも先に両冠動脈，右内胸動脈から肺内に，側副血行を認めた本症候群の稀有な1例を報告した^12）。今回，左冠動脈および右気管支動脈より肺内への側副血行を認め，色素希釈法にてその血流量を測定しえた大動脈炎症候群の1例を経験したので報告する。

A 41-year-old female with aortitis syndrome developing the collateral circulation from the coronary artery and the right bronchial artery to the lung was reported.

Physical examination revealed the loss of bi-lateral radial pulses and the bruits at bilateral lateral neck and supraclavicular areas. Heart murmur was not audible.
