慢性肺気腫における動肺コンプライアンスの呼吸数依存性について

Frequency dependence of dynamic compliance in chronic pulmonary emphysema

　肺の「伸び（縮み）やすさ」をあらわす指標として，コンプライアンス（CL）の概念がある。実際には，注意深く測定された静肺コンプライアンス（Cst）は肺の「伸びやすさ」を反映するが，動肺コンプライアンス（Cdyn）は必ずしも「伸びやすさ」をそのまま反映するものではない。この理由としては，肺内に時定数time constant，すなわちResistance （R）×Compliance （C）の不均等分布があるとき，Cdynが呼吸数の増加に伴って低下することがあげられる^1）。これをCdynの呼吸数依存性といい，逆にこの現象を検討することが末梢気道および肺胞病変の診断に有力と考えられている^2）。

　さて，肺気腫は"肺胞壁の破壊を伴う終末気管支より末梢の気道（腔）の異常な過膨張"を来す疾患であり^3），病理学的に大別して汎細葉性肺気腫panacinar emphysema （PE）と小葉中心性肺気種centrilobular emphysema （CE）に分けられる。これらPEおよびCEは，換気力学的検索によりその特徴がとらえられることが予測され，また肺内時定数（R×C）分布の様相も異ることが考えられるが，まだその裏付けはなされていない。

Frequency dependence of dynamic pulmonary compliance was quantitatively analyzed in relation to the other functional disturbances and the abnormal findings in magnified selective alveolo-bronchograms (SAB) in sixty patients with pulmonary emphysema. All of the subjects showed marked frequency dependence of dynamic compliance. The ratio of dynamic compliance at 50 cycles per minute to that at 10 cycles per minute (C₅₀/C₁₀) was utilized to assess the magnitude of the severity in uneven distribution of ventilation.
