Intractable Status Asthmaticusに対する積極的人工呼吸管理と全身麻酔の応用—その理論的根拠と実施上の問題点について

　気管支喘息の発作中の死亡は近年増加していると言われる^1,2）。しかも発作中の死亡例の大部分は，気管支れん縮，粘膜浮腫および粘液栓閉塞などによる窒息が死因とされている^3,4）。従来のいかなる薬物療法にも反応しなくなり，放置すれば生命の危険のある，いわゆるintractable status asthmaticusに対する気管内挿管JPPBおよび気管支内洗滌法（tracheobronchial lavage）などの積極的呼吸管理法^5-7）は，本邦においてもここ数年，主として麻酔科領域から普及しつつある^8-10）。

　これら積極的人工呼吸管理によっても寛解しない重積発作に対する全身麻酔の応用に関しては，エーテル^11-14）やハロセン^7,14-16）の使用の報告が若干あるのみである。

Intractable status asthmaticus is a critical state which requires agressive therapies to avoid asphyxia. Alveolar ventilation is markedly impaired by the narrowing of the bronchi due to spasm, edema and mucus plugs. Therefore, it might be said that vigorous respiratory care including endotracheal intubation and tracheobronchial lavage is rational for this condition.

From the viewpoint of respiratory mechanics, dyspnea in asthmatic attack is reported more prominent in the inspiratory phase rather than in expiration.
