The gastrointestinal tract is a major target organ of infectious disease. Biopsy plays a key role in determining the infectious agents. Some infectious agents show tissue tropism in epithelial cells and/or mesenchymal cells. Therefore, sufficient tissue should be removed for histopathological examination. A systematic diagnostic approach(e.g., special and immunohistochemical staining methods)should lead to accurate diagnosis. In severely immunodeficient patients(e.g., patients with AIDS or patients undergoing immunosuppressive therapy or chemotherapy), inflammatory patterns are usually atypical and unreliable, and they are consistent with the pauci-inflammatory pattern. Additionally, the possibility of coexisting infections should always be considered. A more thorough patient history(e.g., including their birthplace, recent travels, underlying disease, and medication)and further laboratory data should lead to a better understanding of such cases.
