Gastrointestinal lymphoproliferative disorders are commonly encountered by pathologists, and can pose considerable problems in histologic diagnosis, especially in small-sized biopsy specimens. Small-sized biopsy specimens are subject to sampling errors and artifactual distortion, and on occasion it may be necessary to request a second biopsy or endoscopic mucosal resection to enable a more complete examination. Differentiation of lymphoid neoplasia from reactive lymphoid hyperplasia may be difficult, particularly when the small lymphocytes do not exhibit any evident cytologic atypia. Correlation of morphologic features with a clinical history and results of ancillary studies, such as immunohistochemistry and genetic analysis, may be helpful in differentiating benign lymphoproliferative disorders from malignant lymphomas.
