上部消化管感染症の病理組織学的所見

Clinicopathological Findings of Gastrointestinal Infectious Disease

要旨●上部消化管感染症のうち，組織像にて確定診断ができる単純ヘルペス食道炎，サイトメガロウイルス感染症，カンジダ症，結核，梅毒，アニサキス症，Whipple病，糞線虫症，ランブル鞭毛虫症，非定型抗酸菌症，イソスポーラ症について，臨床病理学的特徴を概説した．これら疾患の多くは背景の免疫不全状態が関連しており，超高齢社会，またHIV感染者が増加している本邦では，今後増加が予想される．組織学的に診断が容易である疾患もあるが，臨床情報と知識，および経験を踏まえたうえで，注意深く検鏡しなければ診断できない疾患もある．

　This study briefly describes the clinicopathological findings of gastrointestinal infectious diseases, which can be diagnosed by histological findings. These include herpetic esophagitis, cytomegalic inclusion disease, candidiasis, tuberculosis, syphilis, anisakiasis, Whipple disease, strongyloidiasis, Giardia lamblia infection, atypical mycobacterial infection, and isosporiasis. Gastrointestinal infectious diseases are associated with immunodeficiency ; therefore, there is an increasing incidence in Japan due to the increase in super-aged societies and patients with HIV. Clinical information, knowledge, and experience are important in the diagnosis of gastrointestinal infectious diseases.
