要旨●近年,細胞のin vivo観察を可能とする2つの超拡大内視鏡検査が臨床応用されており,注目を集めている.すなわち,Endocyto(CF-H290ECI,Olympus社製)とCLE(Cellvizio®,Mauna Kea Technologies社製)の2つである.Endocytoはスコープ先端に光学顕微観察用レンズを搭載した軟性鏡であり,腺腔と核と血管を520倍率で可視化する.CLEはプローブ先端から発光されるレーザーにより,細胞内に吸収された蛍光色素(フルオレセイン)を介した励起光を画像化し,腺腔と血流を1,000倍率で可視化する.本稿では,2018年2月に発売になったばかりのEndocytoにフォーカスを当て,特徴・使用法・精度について概説する.大腸腫瘍診断においては,Endocytoは既存のpit pattern診断に精度の上乗せ効果が期待できるとともに,診断の確信度を上げる効果が期待できる.
In clinical practice, two types of ultra-magnifying endoscopes are currently being applied for in vivo cellular observation:endocytoscopy(Endocyto ; CF-H290ECI, Olympus Corp, Tokyo)and confocal laser endomicroscopy(CLE ; Cellvizio, Mauna Kea Corp., Paris). The Endocyto technique uses an endoscope with a microscopic lens attached to its tip. It allows the visualization of cellular lumens, nuclei, and vessels with a 520× magnification. On the other hand, CLE consists of probe-based laser endoscope, which analyzes the excitatory light emitted through fluoresce in the mucosa. It allows the visualization of cellular lumens and vessels with a 1000× magnification. The present review is manly focused on Endocyto, which was launched in February, 2018. We present its strengths, methodologies, and diagnostic performance. Endocyto carries additional diagnostic value to the current diagnosis based on pit pattern classification, In additon it can contribute to enhancing diagnostic confidence levels.
