要旨●大腸の色素併用拡大内視鏡所見の基礎と要点を中心に概説した．pit pattern診断を学ぶうえではpitや被覆上皮細胞の成り立ちを解剖学的知見から理解することが重要である．コントラスト法では粘膜表層部における無名溝の存在を理解し，pit所見と混同しないことが求められる．クリスタルバイオレット染色法では，われわれがしばしば遭遇する染色不良所見が，①粘膜下層癌や間質反応の露出，②粘液の被覆，③粘膜上皮の物理的な障害のうち，どのメカニズムで生じているのかを鑑別することが求められる．そのためには拡大内視鏡観察において，①周囲不整pitの存在，②連続性の有無の認識が重要であり，これらの基本認識によりその後のパターン分類の理解をよりいっそう深めることが可能となる．

　We have mainly explained the basics and main point of magnifying chromoendoscopic findings. It is important to understand the composition of pits and epithelial cells from an anatomical viewpoint when learning pit pattern diagnosis. Therefore, we must be able to identify the presence of an innominate groove in the mucosal epithelium and not confuse it with pit findings when using the contrast method with indigocarmine. When the crystal violet staining method is used, it is important to differentiate which mechanisms that often result in poor staining are caused from the following：（1）exposure of submucosal invasive cancer or desmoplastic reaction,（2）mucous coating（mucus cap）, and（3）physical injury to the mucosal epithelium. For this purpose, it is important to identify whether there are irregular pits around the poorly stained area or continuity of pits. This can help us better understand the pit pattern classification by basic recognition.
