 A 70-year-old man was admitted to the department of dermatology in our hospital with an umbilical tumor. Biopsy of the lesion revealed a well-differentiated adenocarcinoma including mucinous components, with immunohistochemical findings of cytokeratin and mucin. Contrast-enhanced abdominal computed tomography and gastrointestinal endoscopy were performed to examine the primary tumor. Finally, gastrointestinal endoscopy revealed advanced gastric cancer. The umbilical metastasis in such a case is referred to as a Sister Mary Joseph's nodule. The patient underwent several combination chemotherapies for 24 months but died 25 months after presentation.
