Small Bowel Capsule Endoscopy

要旨●カプセル内視鏡は従来の内視鏡が到達困難であった深部小腸を観察する目的で開発された．ほぼ同時期に登場したバルーン内視鏡とともに，今や小腸検査の主流となった．近年，Crohn病やアスピリンを含む非ステロイド性抗炎症薬（NSAIDs）起因性小腸傷害が増加傾向であり，それら小腸病変に対する内視鏡検査の需要が高まっている．しかし，カプセル内視鏡の見落としや読影負担の課題も残る．今後は磁気誘導型のカプセル内視鏡の開発や読影支援体制の充実などが期待される．

　SBCE（small bowel capsule endoscopy）has been a breakthrough for endoscopic examination of the entire small bowel, similar to balloon-assisted enteroscopy. Since July 2012, SBCE has also been available for patients with suspected or documented Crohn's disease after eliminating capsule retention by the PillCam^® patency capsule. In this session, we have described the usefulness of the latest version of SBCEs and the method of confirming patency using the patency capsule. Moreover, we have addressed its inherent problems and future perspectives.
