要旨●小腸腫瘍の治療前評価における超音波内視鏡（EUS）の役割は，質的診断と腫瘍深度診断であり，内視鏡治療に直結するため重要である．しかし，小腸腫瘍は上皮性腫瘍，非上皮性腫瘍共に発見時に内視鏡治療になる可能性は低いため，臨床でのEUSの使用は限られる．一方，Helicobacter pylori陰性者が増加する中で，十二指腸腫瘍を発見・治療する機会が増えている．特に表在性非乳頭部十二指腸上皮性腫瘍（SNADET）の治療においては，腫瘍のサイズ，形態，リスク・ベネフィットを考慮して治療法を選択するが，EUSは手技の安全性を確保する上で重要である．本稿では，小腸腫瘍に対する診断・治療について，実臨床で比較的遭遇する疾患を中心に解説する．

　The major roles of EUS（endoscopic ultrasonography）in the evaluation of small bowel tumor are the qualitative diagnosis of lesions and the depth grading of the lesion. These are important because they are directly linked to endoscopic treatment. However, most small bowel tumors are not indicated for endoscopic treatment ; thus, the use of EUS is limited in clinical practice. Recently, there has been an increase in the diagnosis and treatment of duodenal tumors. EUS is useful to ensure procedural safety and selection of the treatment method, such as CSP, EMR, Underwater “EMR” and ESD for SNADET（superficial non-ampullary duodenal epithelial tumor）. In this section, we mainly describe the diagnosis of small bowel tumor diseases, commonly observed in clinical practice, using EUS.
