胃ポリープの分類と鑑別―NBI拡大観察での特徴

Analysis of Micromucosal Structure of Gastric Polyps with Magnifying Narrow Band Imaging

要旨　NBI拡大内視鏡観察を行い，生検またはEMRを行った胃ポリープ113例の表面構造とその背景粘膜をfoveola typeとgroove typeに分類し，その関連を検討した．ポリープと背景粘膜の表面構造の一致率は，胃底腺ポリープで94％（16/17例），過形成性ポリープで94％（34/36例）と高率に一致していた．胃ポリープのNBI拡大観察において，背景粘膜に萎縮がなく整った小型円形の腺開口部を認め，ポリープも同様の表面構造であった場合，94％が胃底腺ポリープであった．また，萎縮のある背景粘膜で，ポリープと背景粘膜の表面構造が同様のtypeで，かつポリープの表面に窩間部の開大を認めた場合，94％と高い確率で過形成性ポリープであった．

　We investigated the relationship of micro-surface structure gastric polyp and background mucosa with magnifying NBI（narrow band imaging）in 36 patients with hyperplastic polyp and 17 with fundic gland polyp. The micro-surface structure was classified into foveola and groove type according to whether subepithelial capillaries encircled the gastric pits or not. In magnifying NBI images, the fundic gland polyp had regularly arranged round gastric pits on the surface that were similar to the background mucosa. Hyperplastic polyps showed both foveola（n＝18）and groove（n＝18）type micro-surface structure. Although the intervening parts of the gastric pits enlarged, the micro-surface pattern of hyperplastic polyps remained similar to the background mucosa. The concordance rate of the micro-surface structure between the polyps and the background mucosa was 94.1％ in the fundic gland polyp and 94.4％ in the hyperplastic polyp.
