幽門輪に近接した限局性Ⅱc型早期胃癌の1例

A Case of Ⅱc Type Early Gastric Cancer Localized near the Pylorus

　本症例は幽門輪附近のⅡc型早期胃癌であるが，共存した胃角部の大きな潰瘍性変化のために見直されたもので，それまでに少なくとも2年以上を経過したと思われる．

　The patient is a 64-year-old male, who visited our clinic with a complaint of epigastric pain.

　The x-ray and endoscopic examination of the stomach were performed about Two years ago, then a small ulcer on the posterior wall near the pylorus was found. Two years later, when he visited our clinic for another large ulcer on the lesser curvature of the corpus, this same ulcer near the pylorus was still recognized in its size and moreover internal treatment for two months was ineffective, contrary to the rapid recovery of the large ulcer on the lesser curvature.

　As biopsy specimen showed cancer, gastric resection was performed. In spite of the progress over 2 years, this Ⅱc type cancer localized near the pylorus was histologically adenocarcinoma tubulare limited to the submucosal layer.
