十二指腸球部から第2部に発生した多発性Brunner腺過形成の1例

Multiple Hyperplasia of Brunner's Gland in the Bulb and the Second Portion of the Duodenum, Report of a Case

要旨　患者は51歳，男性．7年前から末期慢性腎不全と診断され血液維持透析中であった．胃X線・内視鏡検査で，十二指腸球部から第2部にかけて発赤・びらんを伴う類円形の大小不同の隆起性病変が多発していた．strip biopsyと生検でBrunner腺過形成の確定診断を得た．現在までに本邦でのBrunner腺過形成の報告は多数あるが，このような形態および分布を示す症例はまれであり，その特徴像と成因について文献的考察を加えた．

　A 51-year-old man, having been on a 7 year period of hemodialysis due to end stage renal disease, was found to have multiple oval-shaped elevated lesions of various sizes with erosions and erythema by upper gastrointestinal x-ray and endoscopic examinations. Strip and regular biopsy specimens revealed Brunner's gland hyperplasia. There have been many reports of Brunner's gland hyperplasia, however, this case was rare in a sense of shape and distribution of the lesions. We discussed the characteristics and cause of Brunner's gland hyperplasia by reviewing the literature.
