炎症性腸疾患の生検標本を用いた臨床研究―炎症性腸疾患における局所免疫異常の解析

Study Using Biopsy Specimens from Patients with Inflammatory Bowel Disease: Analysis on Local Immune Abnormality

要旨　炎症性腸疾患（inflammatory bowel disease；IBD）の病変部粘膜では，T細胞，B細胞，マクロファージなどの免疫系細胞の活性化とサイトカイン，ケモカイン産生の異常，さらにはT細胞および好中球のアポトーシスの異常がみられ，病変の進展および難治化に密接に関係しているものと考えられる．生検標本を用いたIBDの局所免疫に関する研究では同一症例において経時的な変化を検討することが可能であり，長期の経過をとるIBDの病態を解明する上で重要な手段になりうると考えられる．

　The local immune abnormality in inflammatory bowel disease includes activation of T-lymphocytes, B-lymphocytes, and macrophages, aberrant or deficient expression of cytokines and chemokines, and the delay of apoptotic process in T-lymphocytes and neutrophils. These events may be closely linked to the extension and refractoriness of the disease. The analysis of immunological parameters using biopsy specimens obtained at colonoscopic examination enables us to understand the chronologic changes in local immune abnormality in inflammatory bowel disease, which is a long-lasting inflammatory disease of the intestine.
